I am having a heckuva time trying to remove banding from the blue sky in a landscape photo (OK, I confess, the banding was created when shooting Velvia with a polarizing filter). The drum scan was 16-bit. I have read through the archive of posts on this (some of them mine), applied Noise in minimal amounts (per Jeff Schewe), then Gausian Blurs, etc., but to no avail. The image can be viewed at the following URL on my web site. The image on the web may not exhibit the banding, but believe me, it is there!
